ORDINANCE NO. 94-7- S i4 �
AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR TO
EXECUTE CHANGE ORDER NO. 6 IN THE AMOUNT OF $1,400.00 WITH
MIDSTATES CONSTRUCTION INC. FOR RENOVATIONS OF THE LEISURE
SERVICES CENTER
WHEREAS, by Ordinance No. 93-10-5027 adopted October
12, 1993, the City entered into a contract in the amount of
$391,399.00 with Midstates Construction Inc. for renovation of
the Leisure Services Center; and
WHEREAS, Change Order No. 1 in the amount of
$52,855.95 was approved by Ordinance No. 94-1-5062 on January
11, 1994; and
WHEREAS, Change Order No. 2 in the amount of
$22,069.03 was approved by Ordinance No. 94-2-5073 on February
8, 1994; and
WHEREAS, Change Orders No. 3 and 4 in the amounts of
$4,946.00 and $8,247.36, respectively, were approved by
Ordinance no. 94-4-5901 on April 51 1994; and
WHEREAS, Change Order No. 5 in the amount of
$8,055.11 was approved by Ordinance No. 94-6-5131 on June 14,
1994; and
WHEREAS, Change Order No. 6 is required to correct
front entry door to meet code reqirements of the City.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The Mayor is hereby authorized and
directed to execute Change Order No. 6 in the amount of
$1,400.00 with Midstates Construction Inc. for correcting
aluminum front entry door to lobby area of the shops to meet
code requirements of the City. This Change Order is attached
hereto and incorporated by reference as if fully set out
herein.
SECTION 2. The amount of the contract with Midstates
Construction Inc. is amended to $488,498.35.
SECTION 3. This expenditure shall be charged to
Account No.
SECTION 4. This ordinance shall be read on two
separate days and will become effective upon summary
publication pursuant to KRS Chapter 424.
