ORDINANCE NO. 94 -:%-
AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR TO
EXECUTE CHANGE ORDERS NO. 3 AND 4 IN THE AMOUNTS OF $4,946.00
AND $8,247.36, RESPECTIVELY, WITH MIDSTATES CONSTRUCTION INC.
FOR RENOVATIONS OF THE LEISURE SERVICES CENTER
WHEREAS, by Ordinance No. 93-10-5027 adopted October
12, 1993, the City entered into a contract in the amount of
$391,399.00 with Midstates Construction Inc. for renovation of
the Leisure Services Center; and
WHEREAS, Change Order No. 1 in the amount of
$52,855.95 was approved by Ordinance No. 94-1-5062 on January
11, 1994; and
WHEREAS, Change Order No. 2 in the amount of
$22,069.03 was approved by Ordinance No. 94-2-5073 on February
8, 1994; and
WHEREAS, Change Orders No. 3 and 4 are required for
additional improvements and repairs not included in the
original contract.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The Mayor is hereby authorized and
directed to execute Change Orders No. 3 and 4 in the amounts of
$4,946.00 and $8,247.36, respectively, with Midstates
Construction Inc. for improvements and repairs to the Leisure

Services Center which were not included in the original
contract. These Change Orders are attached hereto and
incorporated by reference as if fully set out herein.
SECTION 2. The amount of the contract with Midstates
Construction Inc. is amended to $479,043.24.
SECTION 3. This expenditure shall be charged to
Account No.
SECTION 4. This ordinance shall be read on two
separate days and will become effective upon summary
publication pursuant to KRS Chapter 424.
