AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR
TO EXECUTE CHANGE ORDERS NO. 1 AND 2 IN THE AMOUNTS OF
$2,380.28 AND $242.00 RESPECTIVELY, WITH MIDSTATES
CONSTRUCTION INC. FOR CONSTRUCTION OF A DAY CARE CENTER
WHEREAS, by Ordinance No. 94-3-5088, adopted
March 22, 1994, the City approved a contract in the amount
of $259,850 with Midstates Construction Inc. for construc-
tion of a day care center; and
WHEREAS, Change Orders No. 1 and 2 in the amounts
of $2,380.28 and $242.00 respectively, are required for
additional materials and labor required to complete this
project.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The Mayor is hereby authorized and
directed to enter into Change Orders No. 1 and 2 in the
amounts of $2,380.28 and $242.00 respectively, with
Midstates Construction Inc. for additional materials and
labor required for completion of a day care center. These
Change Orders are attached hereto and incorporated by
reference as if fully set out herein.
SECTION 2. The amount of the contract with
Midstates Construction Inc. is amended to $262,472.28.
SECTION 3. This expenditure shall be charged to
Account No. 55-33-05-470.
SECTION 3. This ordinance will be read on two
separate days and will become effective upon summary
publication pursuant to KRS Chapter 424.
