AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR TO
EXECUTE CHANGE ORDER NO.2 WITH OTIS ELEVATOR COMPANY FOR THE
REPLACEMENT OF THE CITY HALL ELEVATOR
WHEREAS, the City approved Ordinance No. 98-9-5938 to enter into a contract
with OTIS Elevator Company in the amount of $58,740.00 to replace the City Hall Elevator; and
WHEREAS, Change Order No. 1 was required for an increase in the amount of
$3,060.00 for extra equipment rental, additional labor to coordinate change order and complete
installation of elevator, increasing the total contract price to $61,800.00; and
WHEREAS, Change Order No. 2 is required for an increase in the amount of
$1,033.00 to cover Kentucky Use Tax, increasing the total contract price to $62,833.00.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The Mayor is hereby authorized and directed to execute Change
Order No. 2 for an increase in the amount of $1,033.00 with OTIS Elevator Company to cover
the Kentucky Use Tax, increasing the total contract price to $62,833.00.
SECTION 2. This expenditure shall be charged to Account No. 040-8827-536-
2307 (PF0012).

SECTION 3. This ordinance shall become effective upon summary publication
pursuant to KRS Chapter 424.
