AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR
TO EXECUTE AN AGREEMENT BETWEEN THE CITY OF PADUCAH AND
BAPTIST FAMILY FITNESS
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the Mayor is hereby authorized and directed to execute
an agreement between the City of Paducah and Baptist Healthcare System, Inc.d/b/a
Baptist Family Fitness for providing fitness services as part of the City's Wellness
Program. The City will be billed in the amount of $27.50 per participant, maximum
$74.00 per family. Each participating employee will reimburse the City $13.75 per
month, $13.75 for each participating spouse and full cost for each additional family
member thereafter as outlined in said agreement.
SECTION 2. Said agreement authorized in Section 1 above shall be in
full force and effect beginning May 1, 2004, through the remainder of the calendar year
and shall automatically be renewed for additional terms of one (1) year each beginning
January 1, 2005, unless terminated.
SECTION 3. This expenditure shall be charged to the Services — other
account.
SECTION 4. This ordinance shall be read on two separate days and will
become effective upon summary publication pursuant to KRS Chapter 424.
