AN ORDINANCE AUTHORIZING AND DIRECTING THE MAYOR
TO EXECUTE A MEMORANDUM FOR A CONTRACT EXTENSION FOR THE
WELLNESS PROGRAM AGREEMENT BETWEEN KC WELLNESS AND THE
CITY OF PAD
WHEREAS, the City approved Ordinance No. 2002-8-6553 to enter into a
contract with KC Wellness in the amount of $23,813.00 for the Wellness Program; and
WHEREAS, a Memorandum to extend the contract from March 31, 2004,
to December 31, 2004, is required to complete the fourth year of the wellness program
services for the City of Paducah.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the Mayor is hereby authorized and directed to execute
a memorandum extending the date for the Wellness Program Agreement between KC
Wellness and the City of Paducah to December 31, 2004.
