AN ORDINANCE APPROVING A CONTRACT WITH EMPLOYEE
BENEFITS CLAIMS OF WISCONSIN (EBC) FOR ADMINISTRATION OF THE
GROUP HEALTH INSURANCE PLAN FOR THE CITY OF PADUCAH,
ACCEPTING RENEWAL RATES FROM AGENCIES INCLUDED WITHIN THE
HEALTH INSURANCE PLAN, AND AUTHORIZING THE MAYOR TO EXECUTE
ALL DOCUMENTS RELATING TO SAME
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah approves a contract with
Employee Benefit Claims of Wisconsin (EBC), to provide health insurance third party
administrative services effective January 1, 2005.
SECTION 2. That the City of Paducah accepts renewal rates, negotiated
annually, for services provided by Care Continuum, Merck Medco, CenterCare, Symetra
(formerly Safeco) partnered with EBC as part of the health insurance system and these
rates are set out as follows:

SECTION 3. That the Mayor is hereby authorized to execute all
documents relating to same with EBC for services authorized in Section 1 above.

SECTION 4. In order to secure the City's stop loss coverage with
Symetra (formerly Safeco) effective January 1, 2005, an emergency is hereby declared to
exist and this ordinance shall become effective immediately with one reading.
