HomeMy WebLinkAbout2005-1-6905ORDINANCE NO. 2005-1-6905 AN ORDINANCE APPROVING A CONTRACT WITH EMPLOYEE BENEFITS CLAIMS OF WISCONSIN (EBC) FOR ADMINISTRATION OF THE GROUP HEALTH INSURANCE PLAN FOR THE CITY OF PADUCAH, ACCEPTING RENEWAL RATES FROM AGENCIES INCLUDED WITHIN THE HEALTH INSURANCE PLAN, AND AUTHORIZING THE MAYOR TO EXECUTE ALL DOCUMENTS RELATING TO SAME BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. That the City of Paducah approves a contract with Employee Benefit Claims of Wisconsin (EBC), to provide health insurance third party administrative services effective January 1, 2005. SECTION 2. That the City of Paducah accepts renewal rates, negotiated annually, for services provided by Care Continuum, Merck Medco, CenterCare, Symetra (formerly Safeco) partnered with EBC as part of the health insurance system and these rates are set out as follows: Description Basis Medical Admin. (EBC) $13.00 pepm Dental/Vision Admin. ($3.00 Dental & $.50 $3.50 pepm Vision) COBRA (PHFG) $0 pepm Merck Medco — Prescription Drug Carrier $.60/Rx PPO (CenterCare PPO w/Beechstreet — Out of Area Network) ($2.75 CenterCare, $.75 $4.25 pepm Beechstreet & $.75 EBC for repricing) Utilization Review (CareContinuum) $2.65 pepm $10.90 EE Specific Stop -Loss (Symetra) $23.77 ES $150,000 deductible "Paid" TLO $17.88 EC $32.60 ESC Aggregate Premium - $18,338 (Paid $347.72 EE Contract) $758.04 ES Aggregate Claims Attachment Factors $570.27 EC "Paid" agg 13% run-in limit $1039.69 ESC EBC Online Fee (EBC) Included in claims administration fee Claims Liaison Fee (EBC) $4,500.00 SECTION 3. That the Mayor is hereby authorized to execute all documents relating to same with EBC for services authorized in Section 1 above. 293 294 SECTION 4. In order to secure the City's stop loss coverage with Symetra (formerly Safeco) effective January 1, 2005, an emergency is hereby declared to exist and this ordinance shall become effective immediately with one reading.
