A MUNICIPAL ORDER ADOPTING A MEDICAL
REIMBURSEMENT BENEFIT PROGRAM FOR INDEPENDENT FULL-TIME
CITY EMPLOYEES COVERED BY THE CITY'S HEALTH CARE PLAN
WHEREAS, the Board of Commissioners of the City of Paducah
authorized funds for a medical reimbursement benefit program for independent full-time
city employees covered under the City's Health Care Plan; and
WHEREAS, funds for this program were included in the City's FY 1998-
99 budget.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah hereby adopts the following Medical
Reimbursement Benefit Program for city employees not represented by a bargaining unit:
MEDICAL REIMBURSEMENT BENEFIT
"Effective retroactive to July 1, 1998, the maximum each full-time
employee covered under the City's Health Care Plan, Anthem Option 2000, will pay for
allowable expenses per calendar year per covered individual is the first $300.00 of
deductible and/or co -pay. The reimbursement provisions shall be for covered expenses
incurred retroactive to January 1, 1998. The City will reimburse each full-time employee
participating in the Plan for any deductibles and/or co -pay amounts charged over $300.00
per calendar year per covered individual under the Plan. The City's cost will not exceed
$500.00 per covered individual per calendar year, or $1,500.00 per family plan.
"The City will not reimburse any expenses paid by any other insurance
carrier, including Worker's Compensation, or any other medical expenses which are not
considered covered medical expenses under the Plan, or which are not considered as in -
network, usual, reasonable and customary expenses.
"Employees will not be eligible for the City's reimbursement of the
deductible and/or co -payment until they have been employed by the City of Paducah for
one (1) year.
"The City's practice of reimbursing medical expenses exceeding the first
$300 of deductible and/or co -pay is subject to availability of funds and periodic review.
This practice may be discontinued at the discretion of the Board of Commissioners."
SECTION 2. This Order shall be in full force and effect from and after the
date of its adoption.
