MUNICIPAL ORDER NO. 848
A MUNICIPAL ORDER APPROVING A DISEASE MANAGEMENT
PROGRAM FOR FULL TIME EMPLOYEES OF THE CITY OF PADUCAH
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
\) SECTION 1. The City of Paducah adopts the following Disease
Management Program for full time employees of the City of Paducah:
Care Continuum of Louisville can facilitate additional Large Case
Management studies to assist in reduction of cost of services,
streamline patient/provider care and to improve end care of employee
and family participants. The cost of the service adds .45 per month per
contract to the Utilization Review. This move will also necessitate the
change from EBC Review to Care Continuum. New I.D. Cards will be
distributed to each participant free of charge beginning July 1, 2000.
2. Through affiliates of Care Continuum of Louisville, a Nurse HelpLine
can be made available to each participant at a cost of $1.00 per month
per participant. This service brings peace of mind to participants by
way of allowing them to call a R.N. 24 hours per day if they have
medical questions about their personal or family health care.
3. A health assessment should be performed on each participant within the
plan by a specialty firm to draw conclusions as to measures and other
important steps that may be taken in the near future to improve the
health of the employees and families covered by the plan. This will
assist with a benchmark to compare the state of the health of the plan
as a whole and continue to promote ideas of wellness. The cost of this
program is estimated to be $14.00 per participant for a Non-R.N.
review or $24.00 per participant for a R.N. review.
SECTION 2. This program will become effective July 1, 2000.
SECTION 3. This order shall be in full force and effect from and after the date of
its adoption.
