A MUNICIPAL ORDER ADOPTING AMENDMENTS TO PLAN C
AND CHANGES IN COST ALLOCATION TO PARTICIPANTS FOR THE MEDICAL
AND PRESCRIPTION DRUG PLAN OF THE COMPREHENSIVE HEALTH
INSURANCE BENEFIT PLAN SUMMARY FOR FULL TMIE EMPLOYEES OF THE
CITY OF PADUCAH
WHEREAS, by Municipal Order No. 799, adopted by the City of Paducah

on April 13, 1999, the City of Paducah adopted a Comprehensive Health Insurance
Benefit Plan Summary for full time employees of the City of Paducah; and
WHEREAS, the City of Paducah desires to amend this plan at this time.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah adopts the following amendments
to Plan C of the Medical and Prescription Drug Plan Document:
Increase Co -Pay for physician office visits from $5.00 to $10.00 per
visit;
2. Increase Co -Pay for generic prescriptions for 30 -day supply from
$5.00 to $10.00 each; increase Co -Pay for generic prescriptions for
90 -day supply from $10.00 to $20.00 each;
3. Increase Co -Pay for name brand prescriptions for 30 -day supply
from $10.00 to $20.00 each; increase Co -Pay for name brand
prescriptions for 90 -day supply from $20.00 to $40.00 each.

SECTION 2. The City of Paducah adopts changes in cost allocation to
participants for the medical and prescription drug plan.
SECTION 3. The Mayor is hereby authorized to execute documents
authorized in Sections 1 and 2 above.
SECTION 4. These amendments will become effective July 1, 2000.
SECTION 5. This order shall be in full force and effect from and after the
date of its adoption.
