MUNICIPAL ORDER NO. 1292
A MUNICIPAL ORDER ADOPTING A REVISED AND RESTATED
HEALTH INSURANCE MASTER PLAN DATED JUNE 1, 2007 FOR MEDICAL,
PRESCRIPTION DRUG CARD, VISION AND DENTAL PROTECTION FOR
EMPLOYEES OF THE CITY OF PADUCAH
WHEREAS, Municipal Order No. 799 adopted a Master Plan health
insurance benefit summary effective July 1, 1999, administered by EBC of Wisconsin for
full time employees of the City of Paducah; and,
WHEREAS, Municipal Order No. 1077 adopted a Revised Health
Insurance Master Plan, administered by EBC of Wisconsin for medical, prescription drug
card, vision, and dental protection for employees of the City of Paducah effective January
1, 2004; and,
WHEREAS, since that time, nine amendments have been made to the
Revised Health Insurance Master Plan and the City is now desirous to incorporate those
amendments into a Revised and Restated Master Plan dated June 1, 2007
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah adopts a Revised and Restated
Health Insurance Master Plan for medical, prescription drug card, vision and dental
protection plans for employees of the City of Paducah dated June 1, 2007.
SECTION 2. This Order will be in full force and effect from and after the
date of its adoption.
