A MUNICIPAL ORDER ADOPTING AMENDMENT NO. lA TO THE
REVISED HEALTH INSURANCE MASTER PLAN DOCUMENT FOR EMPLOYEES
OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 1077, adopted by the City of

Paducah on June 22, 2004, the City of Paducah adopted a Revised Health Insurance
Master Plan Document for employees of the City of Paducah, and
WHEREAS, the City desires to update this plan in order to increase the
Individual Lifetime Maximum Payment Amount, effective January 1, 2004, and to make
the plan in agreement with the Symetra (formerly Safeco) Life Insurance excess loss
policy adopted by Ordinance No. 2003-11-6717.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 1 to the
Revised Health Insurance Master Plan Document for employees of the City of Paducah.
This amendment is to increase the Individual Lifetime Maximum Payment Amount from
$2,000,000 to $5,000,000 effective January 1, 2004.
SECTION 2. This order shall be in full force and effect from and after the
date of its adoption.
