A MUNICIPAL ORDER ADOPTING A REVISED HEALTH
INSURANCE MASTER PLAN DOCUMENT FOR MEDICAL, PRESCRIPTION
DRUG CARD, VISION AND DENTAL PROTECTION PLANS FOR EMPLOYEES
OF THE CITY OF PADUCAH
WHEREAS, Municipal Order No. 799 adopted a Master Plan health
insurance benefit summary effective July 1, 1999, administered by EBC of Wisconsin for
full time employees of the City of Paducah; and,
WHEREAS, since that time, thirteen amendments have been made to the
Master Plan and the City is now desirous to incorporate those amendments into a Revised
Master Plan Document.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah adopts a Revised Health Insurance
Master Plan Document for medical, prescription drug card, vision and dental protection
plans for employees of the City of Paducah effective January 1, 2004.
SECTION 2. This Order will be in full force and effect from and after the
date of its adoption.
