A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 13 TO THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN SUMMARY FOR
EMPLOYEES OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 799, adopted by the City of Paducah
on April 13, 1999, the City of Paducah adopted a Comprehensive Health Insurance
Benefit Plan Summary for f illtime employees of the City of Paducah, and
WHEREAS, the City desires to update this plan in order to change the
City's Preferred Provider Organization to Center Care, effective January 1, 2004.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 13 to the
Comprehensive Health Insurance Benefit Plan Summary for Employees of the City of
Paducah. This amendment is to change the City's Preferred Provider Organization to
Center Care, effective January 1, 2004
SECTION 2. This order shall be in full force and effect from and after the
date of its adoption.
