HomeMy WebLinkAboutMO # 1042MUNICIPAL ORDER NO. 1042 A MUNICIPAL ORDER IMPLEMENTING A WELLNESS PROGRAM WITH REVISIONS FOR FULL-TIME EMPLOYEES AND SPOUSES OF THE CITY OF PADUCAH BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. That the City of Paducah hereby implements a Wellness Program with revisions for full-time employees and spouses. The Wellness Program with revisions is attached hereto and incorporated by reference as if fully set forth herein. SECTION 2. Brock, City Clerk, February 24, 2004 \mo\wellness program extension 241 CITY OF PADUCAH WELLNESS PROGRAM Eligibility: All full-time employees and their spouses are eligible to participate in the "City of Paducah Wellness Program". Program Guidelines: 242 Completion of a Yearly Physical AND completion of a Health Risk Assessment: Participating employees and their spouses must complete a yearly physical with their Primary Care Physician and submit a Physician Certification Form for verification of their physical date and return it to the Human Resources Department. Participating employees and their spouses must also complete a yearly Health Risk Assessment (HRA), including lab values from their yearly physical, and submit, confidentially, to the agency identified by contract for analysis and individual reporting. This is the primary component of the program, which must be completed in order to be eligible for incentive dollars outlined below. • Physical Fitness: Participating employees and their spouses have a choice of participation at the Fitness Center of their choice an average of eight (8) visits per month, participation in the 10,000 Step Program for twelve (12) weeks or Fitness Quest Program for 12 weeks or participation in 3 of 4 designated 5K Run/Fitness Walks to qualify for incentive dollars outlined below. • Health Education Seminars: Participating employees and their spouses may attend their choice of three health education seminars identified annually which include topics related to medical consumerism, heart health and disease management. Bonus: Participating emolloyees and their spouses may earn additional incentives by participating in extra curriculum activities such as a weight loss program, smoking cessation or other health oriented events designed to promote safety and good health. Other health oriented events must be approved and be equivalent in duration as compared to the requirements of the Physical Fitness Category described above. Participants must complete the program in its entirety in order to qualify for incentive dollars outlined below. Program Incentives: 50 points total = $125 (Primary Award) Complete an Annual Routine Physical and Health Risk Assessment (HRA) 20 points = $50 Fitness Center Participation on average 8 visits per month, participate in the 10,000 step program for 12 weeks, or participate in 3 of 4 of the 5K run/fitness walks 10 points = $25 Health Education Seminar #1 (Identified annually 10 points = $25 Health Education Seminar #2 identified annually 10 points = $25 Health Education Seminar #3 identified annually 20 points = $50 (Bonus) Completion of extra curriculum activities such as a weight loss program, smoking cessation, or other health oriented event Total Points =120 jL00
