A MUNICIPAL ORDER ADOPTING PREMIUMS FOR THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN FOR CALENDAR
YEAR 2004 FOR EMPLOYEES OF THE CITY OF PADUCAH
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah adopt the following monthly
health insurance premiums for calendar year 2004:
Plan A Plan B
Employee $475.00 $610.00
Emp/Spouse $645.00 $750.00
Emp/Child(ren) $625.00 $730.00
Emp/Spouse/Child(ren) $750.00 $870.00
SECTION 2. This order shall be in full force and effect from and after the
date of its adoption.
