HomeMy WebLinkAboutMO # 1021In O IN MUNICIPAL ORDER NO. 1021 A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 11 TO THE COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN SUMMARY FOR EMPLOYEES OF THE CITY OF PADUCAH WHEREAS, by Municipal Order No. 799 on April 13, 1999, the City of Paducah adopted a Comprehensive Health Insurance Benefit Plan Summary for fulltime employees of the City of Paducah; and WHEREAS, the City now desires to amend this plan. BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. The City of Paducah adopts Amendment No. 11 to the Comprehensive Health Insurance Benefit Plan Summary for Employees of the City of Paducah. This amendment includes the following changes: 1) Change services for outpatient surgery to be subject to the deductible Change services for Outpatient Surgery to be subject to the deductible. 2) Change item three wording under "Out -of -Pocket maximum does NOT include: Prescription drug co -pays" to "Out -of -Pocket maximum does NOT include: Prescription drug co -payment percentages." 3) Change wording under Optional Dental Plan, Covered Dental Expenses, Major Services to include "Veneers are now covered on all teeth" from previously limited to "similar properties of crowns and pontics placed on or replacing the 10 upper and lower most anterior (front) teeth". 4) Under Dental Exclusions, Item 10 (page 79) is deleted and replaced with "veneers are now covered on all teeth". 5) Change wording in Amendment No. 8 under "Sources and Methods of Contributions to the Plan" from "The employer pays full premium of employees' coverage. The employer and employee share in the cost of premium for dependent coverage" to "The employer and employee share in the cost of the plan. Employer will provide a schedule of the applicable premiums during the initial open enrollment periods". SECTION 2.
