A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 10 TO THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN SUMMARY FOR
EMPLOYEES OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 799 on April 13, 1999, the City of
Paducah adopted a Comprehensive Health Insurance Benefit Plan Summary for fulltime
employees of the City of Paducah; and
WHEREAS, the City now desires to amend this plan.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 10 to the
Comprehensive Health Insurance Benefit Plan Summary for Employees of the City of
Paducah. This amendment includes the following changes:
(1) Change the name of EBC-Mid America to Employee Benefits
Consultants, Inc. effective March 1, 2003.
(2) Include coverage of injury or emergency services obtained outside
of the PPO network under the PPO level of coverage. This action
is retroactive to July 1, 2002.
(3) Include coverage for full-time students who live outside of the
PPO network area under the PPO level of coverage. This action is
retroactive to July 1, 2002.
SECTION 2. This order shall be in full force and effect from and after the
date of its adoption.
