A MUNICIPAL ORDER ADOPTING AMENDMENTS TO THE MEDICAL AND PRESCRIPTION DRUG PLAN OF THE COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN SUMMARY FOR FULL TIME EMPLOYEES OF THE CITY OF PADUCAH BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. That the City of Paducah amends its health insurance program as follows, effective July 1, 2003: a) b) C) d) e) adoption. Amend coverage for out-patient surgery from 100% with no deductible, to the benefit becoming subject to the annual deductible then paid at the normal major medical rate for Plan A of 70/30 up to the out-of-pocket maximum. Amend coverage for Plan B for out-patient surgery from 100% with no deductible, to the benefit becoming subject to the annual deductible then being payable at 100% after the deductible. Amend coverage for Plan A & Plan B relating to prescription drugs to remove the calendar year out-of-pocket maximum limit of $1,000 on drug copays. Amend Plan A & Plan B as well as the S 125 Cafeteria Plan to a short plan year effective July 1, 2003 ending December 31, 2003. Thereafter, January 1" each calendar year ending December 31 each year.
