A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 5A TO THE
REVISED HEALTH INSURANCE MASTER PLAN DOCUMENT FOR EMPLOYEES
OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 1077, adopted by the City of
Paducah on June 22, 2004, the City of Paducah adopted a Revised Health Insurance
Master Plan Document for employees of the City of Paducah, and
WHEREAS, the City desires to update this plan to change the eligibility
date for new employees for enrollment in the City's health insurance plan.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 5A to the
Revised Health Insurance Master Plan Document for employees of the City of Paducah.
This amendment will change the wording of the Plan to allow for routine annual
mammograms benefits incurred after annual $250 Wellcare benefit has been depleted to
be payable up to a maximum benefit of $50.00 per calendar year.
SECTION 2. This change takes effect August 1, 2005.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
