A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 4A TO THE
REVISED HEALTH INSURANCE MASTER PLAN DOCUMENT FOR EMPLOYEES
OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 1077, adopted by the City of
Paducah on June 22, 2004, the City of Paducah adopted a Revised Health Insurance
Master Plan Document for employees of the City of Paducah, and
WHEREAS, the City desires to update this plan to change the eligibility
date for new employees for enrollment in the City's health insurance plan.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 4A to the
Revised Health Insurance Master Plan Document for employees of the City of Paducah.
This amendment adds a provision for the plan to be compliant with federal mandates
entitles employees to change health plan options during an open enrollment period on in
the event of a family status change such as loss coverage, divorce, marriage, adoption,
death or loss of eligibility. It also includes a change in wording so the Plan is compliant
with COBRA.
SECTION 2. This change takes effect July 1, 2005.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
