A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 7A TO THE
REVISED HEALTH INSURANCE MASTER PLAN DOCUMENT FOR EMPLOYEES
OF THE CITY OF PADUCAH
1 WHEREAS, by Municipal Order No. 1077, adopted by the City of
Paducah on June 22, 2004, the City of Paducah adopted a Revised Health Insurance
Master Plan Document for employees of the City of Paducah, and
WHEREAS, the City desires to update this plan to increase the benefit
provided for preventive services and to impose a penalty for non -network hospital
admission.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 7A to the
Revised Health Insurance Master Plan Document for employees of the City of Paducah.
This amendment will increase the benefit provided for preventive services included under
the City's health insurance plan wellcare benefit to 100% up to $400 per calendar year
per covered individual and impose a penalty of a $1,000 co -pay per admission for non -
network hospital admission which will be in addition to the plan's current non -network
benefit.
'~ SECTION 2. This change takes effect January 1, 2006.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
