A MUNICIPAL ORDER ADOPTING AMENDMENT NO. 8A TO THE
REVISED HEALTH INSURANCE MASTER PLAN DOCUMENT FOR EMPLOYEES
OF THE CITY OF PADUCAH
WHEREAS, by Municipal Order No. 1077, adopted by the City of
Paducah on June 22, 2004, the City of Paducah adopted a Revised Health Insurance
Master Plan Document for employees of the City of Paducah, and
WHEREAS, the City desires to update this plan to extend coverage to
include over-the-counter smoking cessation products.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. The City of Paducah adopts Amendment No. 8A to the
Revised Health Insurance Master Plan Document for employees of the City of Paducah.
This amendment will add to Plan A, Plan B and the Qualified High Deductible Health
Plan coverage of all over-the-counter smoking cessation products subject to a $10 co -pay
per produce for generic products and $20 co -pay per product for brand name products.
SECTION 2. This change takes effect January 1, 2006.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
