A MUNICIPAL ORDER MANDATING CITY OF PADUCAH FULL-
TIME EMPLOYEES AND SPOUSES PARTCIPATING IN THE CITY'S HEALTH
INSURANCE PROGRAM COMPLETE A HEALTH RISK ASSESSMENT
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah hereby mandates that all full-time
employees and spouses participating in the City's health insurance plan complete a health
risk assessment.
SECTION 2. This Order shall be in full force and effect from and after
the date of its adoption.
