ORDINANCE NO. 95-8- 3�
AN ORDINANCE APPROVING CHANGE ORDER NO. 3 IN THE
AMOUNT OF $846.73 FOR THE POLICE DEPARTMENT RELOCATION PROJECT
RENOVATION CONTRACT WITH MIDSTATES CONSTRUCTION, INC.
INCREASING THE TOTAL CONTRACT AMOUNT TO A SUM NOT TO EXCEED
$384,823.23, GRANTING A TIME EXTENSION OF 22 DAYS AND
AUTHORIZING THE MAYOR TO EXECUTE SAME
WHEREAS, by Ordinance No. 94-11-5213, the City
approved an ordinance accepting the bid of Midstates
Construction Inc., in the amount of $340,199 for the Police
Department Relocation Project renovation and authorized the
Mayor to execute a contract for same; and
WHEREAS, by Ordinance No. 95-1-5238, the City
approved an ordinance for Change Order No. 1 for the above
referenced contract requesting the total sum due under said
contract be increased by $12,618 and the granting of a time
extension of 12 days; and
WHEREAS, by Ordinance No. 95-1-5240, the City
approved an ordinance amending Ordinance No. 94-11-5213, to
reflect the original contract price in the total amount of
$357,023.36; and
WHEREAS, by Ordinance No. 95-1-5241, the City
approved an ordinance amending Ordinance No. 95-1-5238
approving Change Order No. 1 which increased the total contract
price to $369,641.36; and
WHEREAS, by Ordinance No. 95-4-5264, the City
approved Change Order No. 2 in the amount of $14,335.14 for six
items required for various improvements to the above referenced
contract, which increased the total sum due under said contract
to an amount not to exceed $383,976.50, and the granting of a
time extension of 20 days, thereby, making the new completion
date May 3, 1995; and
WHEREAS, Change Order No. 3 in the amount of $846.73
is required for relocating the sprinkler exterior connection
and granting an additional 22 days extension of time, thereby
making the new completion date May 25, 1995; and
WHEREAS, said request has been approved by the City
Engineer and City Manager.
BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That Change Order No. 3 in the amount of
$846.73 for relocating the sprinkler exterior connection for
the Police Department Relocation Project renovation contract
with Midstates Construction, Inc., increasing the total
contract to an amount not to exceed $384,823.23, and granting a
time extension of 22 days, to May 25, 1995, is hereby approved.

SECTION 2. That the Mayor is hereby authorized to
execute Change Order No. 3 to the aforesaid contract.
SECTION 3. This expenditure shall be charged to
account number 040-1616-521-4205.
SECTION 4. This ordinance shall be read on two
separate days and will become effective upon summary
publication pursuant to KRS Chapter 424.
