MUNICIPAL ORDER NO. 1584
A MUNICIPAL ORDER AMENDING THE CITY OF PADUCAH'S HEALTH
INSURANCE SUMMARY PLAN DESCRIPTION, FOR FULL-TIME EMPLOYEES, TO
CHANGE THE CITY'S THIRD PARTY ADMINISTRATOR FROM HUMANA TO ANTHEM
BLUE CROSS BLUE SHIELD, EFFECTIVE JANUARY 1, 2011
WHEREAS, Municipal Order No. 1384 adopted the Summary Plan Description
effective May 13, 2008, administered by Humana for full time employees of the City of Paducah
which summarizes the City's health insurance plan and applicable federal guidelines necessary to
keep the City's plan in compliance; and,
WHEREAS, Municipal Order No. 1471 adopted on July 14, 2009, amended the
Plan to change the minimum hours worked by an employee to be covered by the City's health
insurance, removed Paducah Area Transit Systems from under the City's health insurance plan,
and added a prescription drug co -pay benefit to the Elite Plan option; and
WHEREAS, Municipal Order No. 1503 adopted December 15, 2009, amended
the Summary Plan Description for the Elite and Investor Plans to reflect that the vision insurance
will no longer be embedded in the medical insurance plan but will be an optional benefit; and
WHEREAS, Municipal Order No. 1531 adopted April 13, 2010, amended the
Summary Plan Description for the Elite and Investor Plans to provide coverage for routine
colonoscopies, effective for the plan year beginning January 1, 2010; and
WHEREAS, the City has changed Third Party Administrators from Humana to
Anthem Blue Cross Blue Shield effective January 1, 2011 and desires to amend the Plan to
include this change.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah hereby approves the amendment to the
Summary Plan Description for the Elite Plan and Investor Plan of the City's Health Insurance
Plan for full-time employees to include the change of Third Party Administrator from Humana to
Anthem Blue Cross Blue Shield effective January 1, 2011.
SECTION 2. The Mayor is hereby authorized to execute the amendment to the
Elite Plan and Investor Plan approved in Section 1 above.
SECTION 3. This Order will be in full force and effect from and after the date of
its adoption.
