A MUNICIPAL ORDER APPROVING AND ADOPTING THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN PREMIUMS FOR
CALENDAR YEAR 2011 INCLUDING PREMIUMS FOR OPTIONAL DENTAL
�✓ AND VISION CARE FOR EMPLOYEES OF THE CITY OF PADUCAH
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah approves and adopts the following
monthly health insurance premiums for calendar year 201 lincluding premiums for
optional dental and vision care for employees:

SECTION 2. That the premiums for the Comprehensive Health Insurance
Benefit Plan for Employees approved and adopted in Section 1 above shall become
effective January 1, 2011.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
