HomeMy WebLinkAbout2009-12-7641ORDINANCE NO. 2009-12-7641 AN ORDINANCE AUTHORIZING THE MAYOR TO EXECUTE A PLAN MANAGEMENT AGREEMENT BETWEEN THE CITY OF PADUCAH, HUMANA INSURANCE COMPANY, AND HUMANADENTAL INSURANCE COMPANY BE IT ORDAINED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. The Mayor is hereby authorized to execute the Plan Management Agreement between the City of Paducah, Humana Insurance Company, and HumanaDental Insurance Company for services as the City's Third Party Administrator for health insurance administration. The effective date of this Agreement is January 1, 2010. This Agreement shall continue for an initial period of one (1) year from the effective date, unless terminated earlier as provided in Article XI of the Plan Management Agreement. This Agreement shall automatically renew for successive additional one-year periods unless it is terminated as provided in Article XI of the Agreement. SECTION 2. Funding for this service will come from the City's Health Insurance Fund. SECTION 3. This ordinance shall be read on two separate days and will become effective upon summary publication pursuant to KRS Chapter 424. Mayo ATTEST: ljl_y��� S. 411W -C. Tammara S.
