A MUNICIPAL ORDER AMENDING THE WELLNESS PROGRAM
FOR FULL-TIME EMPLOYEES AND SPOUSES OF THE CITY OF PADUCAH
EFFECTIVE JANUARY 1, 2010
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah hereby amends the Wellness
Program for full-time employees and spouses to include the following:
1. Non -Tobacco Incentive: Participating employees receive an
incentive if all family members covered by the plan do not use or
have not used tobacco in the 6 -month period prior to becoming
insured under the City's medical plan, or have enrolled in AND
successfully complete the Cooper -Clayton tobacco cessation
programs as evidenced by a certificate of completion.
Participating employees are required to sign and submit to the
Human Resources Division of the Finance Department a Non -
Tobacco Agreement prior to the end of the program year. The
non -tobacco incentive is valued at $900 per household.
SECTION 2. This Order shall be in full force and effect from and after
the date of its adoption.
