HomeMy WebLinkAboutMO # 1503MUNICIPAL ORDER NO. 1503 A MUNICIPAL ORDER AMENDING THE SUMMARY PLAN DESCRIPTION WITH HUMANA, EFFECTIVE JANUARY 1, 2010, FOR THE CITY OF PADUCAH'S HEALTH INSURANCE PLAN FOR FULL-TIME EMPLOYEES WHEREAS, Municipal Order No. 1384 adopted the Summary Plan Description effective May 13, 2008, administered by Humana for full time employees of the City of Paducah which summarizes the City's health insurance plan and applicable federal guidelines necessary to keep the City's plan in compliance; and, WHEREAS, Municipal Order No. 1471 adopted on July 14, 2009, amended the Plan to change the minimum hours worked by an employee to be covered by the City's health insurance, removed Paducah Area Transit Systems from under the City's health insurance plan, and added a prescription drug co -pay benefit to the Elite Plan option; and WHEREAS, it is necessary to amend the Summary Description Plan for the Elite and Investor Plans to reflect that the vision insurance will no longer be embedded in the medical insurance plan but will be an optional benefit. BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. That the City of Paducah hereby amends the Summary Plan Description for the City of Paducah's Health Insurance Plan for full-time employees of the City of Paducah to exclude the vision insurance effective January 1, 2010, but to allow employees who do not do wish to enroll in the City's medical plan elect to enroll in the dental or vision insurance plan.
