HomeMy WebLinkAboutMO # 1413476 MUNICIPAL ORDER NO. 1413 A MUNCIPAL ORDER AMENDING MUNICIPAL ORDER NO. 1409 ENTITLED, "A MUNICIPAL ORDER APPROVING AND ADOPTING PREMIUMS AND REVISIONS FOR THE COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN FOR CALENDAR YEAR 2009 FOR EMPLOYEES OF THE CITY OF BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY: SECTION 1. That the City of Paducah approves and adopts the following monthly health insurance premiums for calendar year 200 1-a NON -TOBACCO USER PREMIUM TOBACCO USER PREMIUM INVESTOR ELITE PLAN CONSUMER Employee INVESTOR ELITE $920 $1082 PLAN CONSUMER Employee $667 $738 Emp/Spouse $845 $1007 Emp/Child(ren) $708 $850 Emp/Spouse/Child(ren) $987 $1177 TOBACCO USER PREMIUM INVESTOR ELITE PLAN CONSUMER Employee $742 $813 Emp/Spouse $920 $1082 Emp/Child(ren) $783 $875 $925 Emp/Spouse/Child(ren) $1062 Q'� $1252 SECTION 2. That the City of Paducah approves and adopts the following changes to the Comprehensive Health Insurance Benefit Plan for Employees of the City of Paducah: 1) Add a Drug Card to the Elite plan. 2) Abandon the City's third plan option, the Enhanced Plan. 3) Increase the co-insurance benefits on the Elite Plan from 900/o/10% to 80%/20%. 4) Increase the City's allocation for health insurance costs to employees who participate in the City's health plan from $7,956 to $8,500. 5) Allow employees to retain the benefit of funds in a Health Reimbursement Account after they leave the City provided they have at least five years tenure. SECTION 3. That the premiums and revisions for the Comprehensive Health Insurance Benefit Plan for Employees approved and adopted in Sections 1 and 2 above shall become effective January 1, 2009.
