A MUNICIPAL ORDER APPROVING AND ADOPTING THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN PREMIUMS FOR
CALENDAR YEAR 2010 AND INCLUDING THE AMENDMENT TO THE PLAN
FOR OPTIONAL DENTAL AND VISION CARE FOR EMPLOYEES OF THE CITY
OF PADUCAH
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah approves and adopts the following
monthly health insurance premiums for calendar year 2010:
SECTION 2. That the City of Paducah approves the amendment of the
Comprehensive Health Insurance Benefit Plan to include a Dental and Vision Care Plan.
The rates are as follows:

SECTION 3. That the premiums and revisions for the Comprehensive
Health Insurance Benefit Plan for Employees approved and adopted in Sections l and 2
above shall become effective January 1, 2010.
SECTION 4. This order shall be in full force and effect from and after the
date of its adoption.
