A MUNICIPAL ORDER APPROVING AND ADOPTING THE
COMPREHENSIVE HEALTH INSURANCE BENEFIT PLAN PREMIUMS FOR
CALENDAR YEAR 2015 INCLUDING PREMIUMS FOR OPTIONAL DENTAL
AND VISION CARE FOR EMPLOYEES OF THE CITY OF PADUCAH
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah approves and adopts the following
monthly health insurance premiums for calendar year 2015 including premiums for
optional dental and vision care for employees:

SECTION 2. That the premiums for the Comprehensive Health Insurance
Benefit Plan for Employees approved and adopted in Section 1 above shall become
effective January 1, 2015.
SECTION 3. This order shall be in full force and effect from and after the
date of its adoption.
