A MUNICIPAL ORDER AMENDING THE SUMMARY PLAN
DESCRIPTION FOR THE CITY OF PADUCAH'S HEALTH INSURANCE PLAN
`' FOR FULL-TIME EMPLOYEES
WHEREAS, Municipal Order No. 1384 adopted the Summary Plan
Description effective May 13, 2008, administered by Humana for full time employees of
the City of Paducah which summarizes the City's health insurance plan and applicable
federal guidelines necessary to keep the City's plan in compliance; and,
WHEREAS, since that time, amendments have been made to the
Summary Plan Description and the City is now desirous to incorporate those
amendments.
BE IT ORDERED BY THE CITY OF PADUCAH, KENTUCKY:
SECTION 1. That the City of Paducah hereby amends the Summary Plan
Description for the City of Paducah's Health Insurance Plan for full-time employees of
the City of Paducah. The amendments are:
• The definition of "employee" is actively performing the principle
duties of his/her occupation a minimum of 30 hours per week" has
been changed to "actively performing the principle duties of
his/her occupation a minimum of 32 hours per week".
Remove Paducah Area Transit Systems as an affiliate under the
City's Health Insurance Plan.
Add a prescription drug co -pay benefit to the Elite Plan option.
A copy of the Summary Plan Description is on file in the City Clerk's office.
SECTION 2. This Order will be in full force and effect from and after the
date of its adoption.
